Reporting Tips  When reporting claims, report them immediately – don’t delay.   Collect as much information as possible regarding the loss such as contact name and phone number, name and address of the church or reporting location, parties involved, address, date and time, policy number, accident description, type of injury.   Submit all police reports, estimates, photos and any materials/receipts.   Do not speak with third parties about the claim, do not discuss “fault”.   If at all possible, complete a claim reporting form.  Below are links to the various forms (by coverage line) for your reporting use.
